MR Group personnel were conducting an internal inspection of the BOP stack of a tender barge drilling unit. While the inspection was proceeding, a section of scaffolding failed whilst one of the inspectors was standing on it.

In this Safety Awareness we adress the dangers and consequenses when temporary access arrangements are not installed properly. API RP 54 is a useful refrence to manage such risks.
